An exciting opportunity has arisen for several Reablement Workers to support our team in the Joint Reablement team Service based in Thurrock, where personalised care and reablement are provided through a multi-disciplinary team including a Physiotherapist, Occupational Therapist, Social Worker and Nurse. We cater for older people with differing needs and those people require extra support with personal care duties.
Joint Reablement Team staff work in a way to promote personalisation and reablement while assisting in achieving the delivery of outcomes for residents within their own home. You will be responsible to support the existing team to follow Individual Support Plans leaning schedules to ensure high standards of cleanliness within the home. You will be expected to promote our core values. The Core values include individuality, independence, choice, dignity, respect, rights, confidentiality and fulfillment, which guarantees users of the service, receives a high standard, quality service.
The type of person who will be suitable for this role will need patience, understanding, be committed and have a genuine caring nature and be able to demonstrate the core values of the service. A good understanding of The Care Act 2014 and Care Quality Commission Compliance.
Ability to work flexibly as well as working evenings and weekends is essential. Full Driving Licence and use of a vehicle essential. Weekend workers are also welcome.
