This is a full time 12 month fixed term contract/secondment covering maternity leave.

Surrey Heath Integrated Care Team is looking for an experienced leader with expert knowledge and skills in social care to be part of its Integrated Care Management Team.
Working alongside two other Team Leaders from health disciplines you will be part of a management team that is interpreting the vision of truly integrated care into operational reality.
Sharing that vision and inspiring other members of staff is critical to the role, so you will need excellent communication, change management and motivation skills.
As the most senior representative of social care we will also expect you to have a robust knowledge of legislation and best practice, as well as the ability to professionally support and line manage other social care staff within the integrated structure.
One of your main responsibilities will be to lead one of three teams based in GP surgeries across Surrey Heath. Each team is made up of health and social care staff from different disciplines including community nursing and older people’s mental health working alongside general practice. Experience of working with these and other health disciplines will be a real asset for the successful candidate. Your role will be to co-ordinate and facilitate effective multi-disciplinary working within the team, part of this will include chairing regular multi-disciplinary team meetings and promoting joint working at every opportunity.
Whilst your formal line management and support will be through Adult Social Care in Surrey County Council, you will also be supported by the Integrated Care Manager and your Team Leader colleagues from other disciplines in managing the integrated care structure.
Integrated Care in Surrey Heath is still in its formative stages so you will an opportunity to shape and develop the direction of travel.
This is an exciting role for someone currently working at a senior level within social care who wishes to develop their skills and knowledge and explore the opportunities that result from working in an integrated way across other disciplines and services.
The integrated care model in Surrey Heath is currently designed to operate between 8am and 8pm Monday to Friday. As such you may be called upon to provide management support between these times as part of your contracted hours.
