The Team

The Independence and Wellbeing Team, which is part of Adult Social Care, aims to improve quality of life, reduce health inequalities and promote independent living amongst vulnerable adults.  Our work focuses on supporting and empowering adults to improve and maintain their health and wellbeing, to enable them to remain living independently in their homes and communities.  We are a diverse multi-disciplinary team with a person centred, evidence based approach and we believe that prevention, workforce development and partnership working is key to our success.

What is the role?

We require a part-time member of staff to work on our Community Connector Service. You will support vulnerable adults, aged 18+, who are lonely or isolated and may be experiencing poor mental health to identify and access resources and opportunities within their local communities which will improve their connectivity and increase their social and support networks. The focus of this service is on early intervention to promote health and wellbeing, help maintain independence and build resilience. Some of our clients have quite complex needs which requires close partnership working across Adult Social Care and other

services.
