MAXIMISING INDEPENDENCE TEAM (Joint Care Provider Services)
Salary Scale £26,470-35,229 depending on experience

West Berkshire Council has a rare opportunity for a full time HCPC registered Occupational Therapist to join our friendly and supportive teams. We are a small Local Authority with a mixture of urban and rural communities which offers great variety and positive challenges. Joint Care Provider Services provides an integrated health and social care pathway for people requiring a care package to facilitate their discharge from hospital. The Occupational Therapist will work closely with individuals once they return home to focus on goal setting and getting people back to their previous level of independence. The Occupational Therapist will work closely with the Physiotherapy team, WBC Reablement carers as well as external care providers in ensuring that situations are safe and person centred. You will work alongside Social Workers and Social Care Practitioners where long term care needs are identified and safeguarding or mental capacity issues have been highlighted.
The role will require you to have knowledge of reablement strategies, equipment provision, moving and positioning risk assessments, single carer management, postural management, minor adaptations. We also have a number of exciting projects ongoing, including the provision of specialist equipment to improve and/or reduce long term care packages

This role has been identified as public facing in accordance with Part 7 of the Immigration Act 2017; the requirement to fulfil all spoken aspects of the role with confidence in English applies.
