Project Manager – Integrated Care
Location: Hertford - covering Countywide
Starting salary: £34,106 pa progressing to £44,697 pa
Hours: 37 hours per week
Contract: Fixed Term contract, 12 months

Could you be the right person to help us deliver our vision of integrated health and social care across Hertfordshire? Could you help to improve the quality of life of our residents by designing and delivering integrated services?
The Integrated Care Programme Team is looking for forward-thinking candidates who have project management, operational and/or commissioning experience and an in-depth knowledge of the health and social care sector. We are seeking people who can build relationships with a wide range of stakeholders and work across organisational boundaries in order to facilitate change.
Hertfordshire is at the forefront of delivering new models of health and social care and a number of our services have won national awards. We have agreed an ambitious vision for integrated care, set out in our Better Care Fund strategy and aligned to the Hertfordshire and West Essex Sustainability and Transformation Plan (STP).
Our Project Managers work on a wide ranging portfolio to deliver this vision for the health and care system. At times this requires us to work anywhere across Hertfordshire, working with a range of partners including the Clinical Commissioning Groups, health and care providers, and our communities including the voluntary sector. Examples of recent transformation projects include:
- Integrating community teams by bringing together health and care professions to provide more person-centred care and reduce the need for admission to hospital;
- Improving hospital discharge services;
- The development of a home improvement agency to deliver home adaptations that keep people living independently at home;
- Enhancing care in our many care homes, including training and care assessments; and
- A digital care programme to create a single shared care record across all agencies.
