East Sussex is currently working towards implementing East Sussex Better Together. This will see the integration of health and social care teams, ensuring that every patient or service user, enjoys proactive joined up care, to support them to live as independently as possible and achieve the best possible outcomes.
East Sussex County Council (ESCC) is looking for Occupational Therapists to be part of this process.
The role will include assessing Adults in-line with the Care Act. As an Occupational Therapist you will have a pivotal role in the well-being agenda and inform the interventions needed to meet clients and carers, assessed eligible needs. You will manage and reduce risks, allowing clients to remain as safe as possible in their own homes, using a re-ablement approach. The role will involve working with a range of daily living equipment, undertaking adaptations, moving and handling and working jointly with partner agencies.
You need to have skills to manage the challenges that an environment of continual improvement and development brings.
