The Assessment and Reablement Team provides support to older adults at a time of significant change in their life. The aim of the service is to support and guide the Reablement Service in North Somerset and to maximise Independence for our service users in North Somerset. We do this by supporting people both in Care and those in the community to regain function or manage functional changes.
This is a new permanent post due to the success of the Assessment and Reablement team in North Somerset.

The team is based in Castlewood, Clevedon, where we are co-located with a variety of other Health and Social Care Teams, giving us an ideal opportunity to network with our colleagues. Your working area will cover North Somerset including the coastal towns of Weston-super-Mare, Portishead, Clevedon as well as the beautiful country-side of North Somerset.
Working together as a team of professionals from Health and Social Care we offer a unique, collaborative service. This approach enables easy and timely access to services as well as helping Organisations to work effectively together in a truly integrated manner.
Your personal and professional development is important to us. As well as regular supervision and in-service training, working in the Assessment and Reablement Team offers an excellent range of training and development opportunities for staff. This will allow you to continue to develop your Occupational Therapy skills as well as a range of other skills and knowledge.
