Role Purpose

To work with local people to help them identify and then access the support they need to maintain and enhance their health and wellbeing. This will include, a collaborative holistic assessment of factors (including social, environmental and behavioural) that influence wellbeing, providing high quality information and guidance on the services and local assets available followed up by proactive support to enable them to access this support.

In undertaking this role the post holder will gain from their interactions with local people valuable insights of their perceptions of services and local assets. Part of the role is to ensure these insights are captured and recorded to enable them to inform the design and commissioning of health and social care services.

The post will be based in an Idea Store but will involve a substantial amount of assertive outreach work to ensure effective interaction with local residents. The post holder will also spend part of their time each week providing information and advice on all public health services within the structures that are put in place to achieve this.



Key Responsibilities

Tocarry out with individuals,collaborative holistic assessment, using the designated tools, of factors(including social, environmental and behavioural) that influence wellbeing.These assessments may take place in an Idea Store, community settings or bytelephone.

To support individual in goal setting and personalplanning in relation to their holistic assessment outputs.

To capture and record community insights gainedfrom interactions with local people, this will include contributing to, andensuring data sources are kept up to date and as comprehensive as possible,including the community directory and local asset maps.


To capture and provide monitoring and evaluationdata as requested

To develop strong relationships with community andvoluntary sector organisations, creating seamless referral and signpostingpathways

To proactively promote the Health Outreach Serviceacross the council and all other partner organisation within the Public andVoluntary & Community sector

To contribute to the Health & WellbeingStrategy of the Idea Store in which the post is based including:

Implementing a regular programme of health relatedevents in the Idea Store as determined by the service need

Participating in the provision and promotion of ahigh quality health information service in both print and digital formats andincluding the TH Wellbeing Wheel and Idea Store online directory

Promoting all Idea Store Learning courses that willhave a positive impact on health and wellbeing, including classes that willlead to employability and family learning classes as well as Fitness, Healthand Wellbeing programmes and ESOL (English for Speakers of Other Languages)

Acting as health champion at a dedicated Idea Storeensuring that all other team members are fully briefed on all health relatedinitiatives

Supportingpeople, especially those with poor computer and internet literacy, in gainingaccess to digital services and online health information. Conducting the majority of the administration in connection with thepost, using a PC and word processing/presentation software as appropriate




Requirements

1. To work as part of the structures that are put inplace to facilitate referral to health and wellbeing services,receiving referrals, conducting telephone assessment, following up clients andcarrying out any required associated administration ensuring that allinteractions are captured on the service database.

2. To attend and organise community events and promotehealth and wellbeing services and activities, either as a local or borough wide.

3. To maintain an up-to-date log of programme activityin order to assess the impact of this service in helping the local communityand also to help shape its future development.


5. To participate in all aspects of the council’sperformance management scheme, ensuring that performance standards and targetsare set and met within agreed timescales.
