Job Duties:

Are  you

  • HCPC registered with a qualification in Social Work
  • Experienced in assessing and managing support for people with learning disabilities and their carers
  • Experienced in managing complex case work in the areas of safeguarding and personalisation
  • Able to travel around and out of the County on a regular basis

About us

0-25 are an innovative service working with children and young people aged 0-25 with disabilities. We are looking for an Advanced Practitioner to work with our young people as they plan for adulthood and up to the age of 25.

As an Advanced Pratitioner you will lead practice in:

  • Adult Safeguarding
  • Our Three Conversation Model of Assessment Asset Based Practice and Building Community Networks
  • 0-25 is a multi disciplinary service and consists of:Adult Social Workers
  • Children Social Workers
  • Preparing for Adulthood workers
  • Family Practitioners
  • Learning Disability Nurses
