Due to the proven success of the team’s work of increasing independence of disabled people in North Somerset and our commitment to Discharge to Assess from the Acute Trusts, we are offering an exciting and unusual employment opportunity within the Access and Hospital Support Team. This is a developing team - we have a wide remit, including working with adults with physical disabilities to increase their independence and allow them to remain in their own home. We work closely with the hospital, community health Services, voluntary sector, independent providers and other Adult Social Services Teams
We are a multi-disciplinary, joint Health and Social care team that is friendly and professional. We are working closely with the Acute Trusts and our community Health Colleagues to facilitate discharge and are developing close links with our partners within the third sector including our local Home from Hospital scheme.
The successful candidate should have established community experience and a good understanding of a wide range of services available throughout North Somerset. You would be working alongside professionals in the team including Social Workers, Occupational Therapists, Physiotherapists and Nurses. An understanding of the Assessment Process, Personalisation and community Services (Voluntary, independent, Health and Social Care) would be an asset. You would be expected to keep accurate records and computer skills are essential.
