Brent Council and CCG, working in partnership with key partners through the joint Health and Care Transformation Board (HCTB), have agreed the following vision:
“To achieve system sustainability in order to be a national exemplar for delivering patient centred care for people through practical front line working, supported by integration and effective joint operational and strategic leadership”
A joint programme of work has been agreed to deliver this vision:
1. Older people’s pathway – improving patient flow out of hospital through the establishment of a single point of access, service alignment and improved processes to ensure more people are discharged home through ‘Home First’.
2. Integrated commissioning and market management – working jointly across social care and CCG funded care to ensure better care for people and joined up discussions with the market
3. Enhanced care in care homes – working with nursing and residential care providers to respond as a system to the challenges facing the sector and reduce pressure on higher acuity health and care services
In addition, the Board have agreed to scope three further areas throughout 18/19:
4. Self care – to determine opportunities for greater management of care in the community, reducing hospital admissions and long term care and support
5. Dementia and challenging behaviours – to explore opportunities to develop services and support for people with dementia and challenging behaviours to improve outcomes for individuals
6. Integrated care system development – to explore the potential for further integration across health and social care, including alignment of incentives to deliver key outcome objectives for Brent health and care system

The post is a jointly funded role between the CCG and Council, as part of the Health and Care Transformation Team, to support delivery of one of three top priorities in Brent’s Health and Care Transformation Plan: Integrated commissioning and market management.
The key areas of focus for the role are:
• To develop a joint understanding of the nursing and residential market, and a joint approach to working with and managing providers to deliver better value and outcomes for commissioners
• To develop greater shared understanding of existing procurement frameworks based on quality and price of placements made, with a view to the development of a shared pricing and procurement strategy, delivering greater harmonisation of price and value
• To develop practical joint working arrangements to support the objectives for market management and integrated commissioning
• To work together to drive up the quality of care home provision, by working together in partnership with providers to manage performance and provide support where required
• To deliver the best possible care for people, regardless of who pays
• To ensure a consistent approach to assessment across health and social care, in line with older people pathway, to reduce duplication
• To rationalise engagement and communication with providers to support the more integrated model
The post will report to the Director of Integrated Care, but will be required to work with senior partners across the CCG and Council.
The candidate will be able to work effectively with senior stakeholders, have strong programme management skills, analytical skills and experience of working across health and social care settings. But above all, the candidate will be hard working and passionate about improving care for people.
The candidate will also need to demonstrate:
• Evidence of Continuous Professional Development.
• Educated to degree level (or evidence of equivalent level of education).
• Detailed knowledge of the specific portfolio.
