Reading Adult Social Care are recruiting Social Care Coordinators to work in our new and exciting Adult Social Care Short Term Team. You will work alongside social workers and occupational therapists and Sensory Needs Officers in a multi-disciplinary team, working with vulnerable adults, including learning disabilities, long term conditions, older people, sensory needs and adults on the autistic spectrum.

Our aim is to help people meet their needs and achieve the outcomes that matter to them in their lives, involving the person and supporting them to have choice and control over what matters to them.

The role:

You will undertake non-complex assessments, reviews and care and support planning with people and their carers, promoting independence and protection.

Enabling people to identify their needs and outcomes, and liaising with other agencies when required.

You will work within the legal framework provided by the statute, guidance, policies and procedures.

We are looking for candidates who have previous experience working with vulnerable people or have transferable skills and experiences.

You must be able to demonstrate:

  • excellent communication skills
  • ability to gather and analyse information
  • ability to identify and prioritise risks
  • ability to identify and prioritise safeguarding issues
  • excellent organisationals skills and flexibility
  • excellent IT and typing skills
  • excellent team work skills.
