We are looking for enthusiastic, committed and creative Assistant Practitioners to join Southwark's Mental Heath Adult Social Care division in the Care and Support team.
The service offers assessment and support to adults whose mental illness and/or impairment restricts their activities of daily living. Our focus is to maximise a person's independence, to offer choices, to promote well being and to meet our duties under the Care Act and s117 . We have the scope to develop creative and innovative approaches to ensure service users are at the centre of the support they receive.
We work closely with primary care, secondary mental health services and third sector providers. Working in collaboration with the other members of the team, social workers and occupational therapists as well as other agencies, you will enable current support plans to be implemented. You will ensure that such plans are personalised, recovery focused and promote independence. You will work directly with adults to coach and enable them to improve their skills, abilities and confidence to be able to carry out their activities of daily living. You would use a strengths based approach, utilising all the resources available in the community to meet the adults identified social care needs.
