Social Care Practitioner
The Brokerage and Professional Support Team provide services to vulnerable adults who have a learning disability, older people and /or a physical disability. The team meet the long-term eligible care and support needs as assessed under the Care Act 2014 of both customers and carers. We strive to enable individuals to live a safe, fulfilling and independent life as possible ensuring their wellbeing is paramount.
We are a busy, productive and supportive team, which consists of Social Workers, Occupational Therapists, Social Care Practitioners a Sensory Needs Service, a Technology Enabled Care Team, a Reviews Team and a Transitions Team. here is considerable join working with colleagues across the teams and a very much can do/ solution-based culture.
We are currently looking for a Full Time Social Care Practitioner to join our team. The Post holder will work within an established multi professional statutory team and will undertake re-assessments for people in the community with long term eligible care and support needs in line with the Care Act (2014) framework. The Post holder will also be completing Support Plans based on an agreed Personal Budget ( PB) and undertake reviews to monitor and revise support in place. You will also undertake Carer Assessments as and when needed.
We are looking for a motivated, caring and creative applicant who will look at ways to enhance outcomes for individuals by working with them taking into account their strengths and views. You will already have some experience of being in contact with Vulnerable Adults.
You will need to be able to work in a busy, fast paced environment, be able to plan and adapt to change. You will need to be flexible and be able to prioritise and manage your time effectively.
You will carry a caseload which will mean undertaking visits to individuals in their own homes and other provisions in the community. You will receive regular supervision and support as necessary and will have a comprehensive Induction period and a “buddy” assigned to help you settle in. There will be regular opportunities to undertake training which will be relevant to equip you undertake the role.
