We are a Local Authority working for the community, improving the lives of our residents and we’re looking for motivated and talented Physiotherapist to join our Discharge to Assess and Reablement Team. We are looking for individuals who can demonstrate great communication skills and the ability to build and maintain relationships with colleagues, services users, and health care providers.
You will work alongside Occupational Therapists, Social workers, care providers and trusted assessors to maximise independence following hospital discharge or a change in need. You will be part of a multidisciplinary team supporting a 4 bedded Discharge To Assess Unit and working with individuals in their own homes.
The role involves working with adults with a broad range of needs in their own homes including but is not limited to deterioration associated with ageing and dementia, hospital acquired functional decline, frailty, and other long-term conditions. Working with in an Adult Social Care setting

About the Role:
You will hold a varied caseload and contribute to the team performance and positive outcomes for our residents.
You will support individuals to remain in their own homes and maximise their independence. Using your physiotherapy skills to support with assessment, and ongoing physiotherapeutic management of adults with varied physical rehabilitative needs.
You will work within the legal framework provided by the statute, guidance, policies, and procedures.
You will need to have:
- A degree in Physiotherapy
- Registered with HCPC
- Community experience
