This is an exciting opportunity to join a team of adult social care professionals! We are looking for an experienced Social Worker to work with us in our Adult Social Care Team providing support and personal care plans to our adult community in Rutland. - Qualified Social Worker registered with Social Work England
- Knowledge of Adult Care legislation and procedures
- Experience of working with adults in a social work or health environment
- Experience of CHC DSTs, mental capacity assessments, DoLs and best interest decision making
About the role…
As a Social Worker in our adult social care service you will be responsible for the following:
- To assess the social care needs of Rutland (adult) residents who are in need of community care services.
- To create, review and monitor support plans in partnership with service users and carers
- Update and maintain case file notes and electronic records
- To work in an integrated way with partners including health and colleagues across LLR and locally
In addition, we can offer manageable caseloads, with reflective peer practice, CPD sessions and regular 1 to 1 supervision.
The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS and Social work England registration are paid by us.
