We are currently looking for a Carer Support Social Worker to work as part of our multi-professional team providing support to patients, carers and families at the end of life, both in the community and on our Inpatient Unit.
You will lead the day-to-day delivery and development of carer support at the hospice. This will include leading on assessments of carer need, possibly recruiting and overseeing volunteers to assist with this task. It will include your own small carer related social work caseload, leading group support for carers, developing relevant information resources and further improving our external partnerships regarding carer support.
