The Transitions Service supports young people from the ages of 16-25 with severe and enduring disabilities and their families in a planned and personalised way.
We have a great opportunity for a motivated and proactive registered Social Worker who is committed to ensuring that the service supports young people and their families to make choices about their future in an individual and timely manner.
To fully support young people as they transition into ASC we work closely with Children’s Disability Social Care, Education and Health Services. Knowledge, experience, and an understanding in this area of social work would be advantageous as will experience of working with young people with disabilities and their families. If you don’t already have this breath of experience, we can train you.

The Transition service is part of a large Council which actively promotes social workers CPD by offering regular training, appraisals, and supervision.
There is likely to be the expectation of working from home in the Covid 19 recovery period. ESCC have adopted a hybrid workstyle. However, you will need to be able to independently travel to cover the catchment area of the team and to fulfil the needs of the role.

Your starting salary will be pro rata if the above position is part-time or term-time only.
Work Permits: we may be able to obtain a Work Permit for this post but this is subject to meeting the requirements of the UK Border Agency’s Points-based Immigration System.
