Starting salary: JG7:- £27,358 / JM1:- £30,756 per annum
Location: Based in Clevedon, and working across North Somerset
Hours: 37 hours per week x 2 posts

An opportunity has arisen for an enthusiastic Social Worker to join our developing and expanding Assessment and Reablement Team. We are a multi-disciplinary, integrated Health and Social Care team and have a wide remit to work with disabled adults.
We are a friendly and professional team who provide care management within the hospital environment to North Somerset residents across the county. The team also provides an Intermediate Care Service, which assists people who have recently moved into care homes to regain or maintain their independence within the community. Furthermore, we are integrated with colleagues from the Health Service (North Somerset Community Partnership).
We would love to hear from you if you have excellent communication skills, are committed to giving disabled adults choice and independence. You need to have the ability to demonstrate good practice, concentrating on time sensitive assessments of need and developing, implementing and monitoring packages of support in partnership with relevant others.
