An excellent opportunity has arisen to join our innovative Portsmouth Rehabilitation and Reablement Team (PRRT).  

We are looking for an enthusiastic and highly motivated Social Worker who will be required to work a shift pattern from 8am to 8pm including one in four weekends.

A proportion of this role involves generic working as part of our community urgent response team which provides opportunity to experience challenging decision making and to develop assessment skills.  

 

The Service

PRRT is a multi-disciplinary team who provide short-term community base intermediate care, rehabilitation and reablement to adults and older people in Portsmouth City during the hours of 7am – 10pm over seven days, to promote independence, improve quality of life and maintain people living at home where they want to be.  

The team consists of Occupational Therapists, Physiotherapists, Nurses, Social Workers and Rehab Assistants.  

 

What is the role? 

You will be an integral member of the multi-disciplinary team and will promote independence, choice and rights and maintain people in the community.  

You will assess the needs of vulnerable adults and their carer’s, and identifying creative and personalised ways to provide support.

You will be working independently and be responsible for managing your own caseload and maintaining case records within an environment offering a high level of supervision and support.  

The safeguarding of adults is an important and integral area of your work as will be the implementation of the safeguarding policy and procedures.  

You will be fully conversant with statutory legislation and guidance, and demonstrate that you implement this in your day to day practice.

You will be assessing people under the Care Act 2014, The Mental Capacity Act 2005 and for Continuing Health Care.

 


What you’ll need to succeed


You need to have:

Be passionate about putting the customer at the heart of what you do, having an understanding of the needs of the client group, promoting and supporting individuals’ rights and choice and have a Social Work qualification and be registered with Social Work England as a Social Worker.

Be highly skilled, experienced, and motivated, committed to learning and maintaining the highest level of professional knowledge and development and be fully conversant with the Care Act, the Mental Capacity Act, Adult Safeguarding legislation, the Personalisation Agenda and other applicable legislation.

A full understanding of, and commitment to, embedding personalisation in all aspects of social care, with the ability to cope with a case workload efficiently and effectively.

Good evidence based recording skills, as your evidence may be required to be used in court.

The ability to work on your own initiative, organise your own work load and work under pressure in response to competing demands and changing priorities.

Experience of working in a in a multi-disciplinary environment.

Have a full driving licence and access to a vehicle for work purposes.
