Adult Care Point 2 Team:

Adult Care Point 2 provides the main front door for adults requiring support from social care. The team consists of occupational therapists, social workers and assessment officers who complete a strengths-based Care Act (2014) phone assessment and work with the person to identify solutions.

Following assessment, advice/information could be provided to support someone to continue to live independently; or a customer could be signposted to other services (both statutory or non-statutory) as well as consideration given to possible aids, equipment, or adaptations.

The role of the Senior Practitioner is to develop and maintain positive working relationships with colleagues from other organisations such as the police, children services and reablement. They provide daily advice and guidance to staff as well as leading on supervision and ensuring all team members are working using a strength's-based approach.

Mental Health Team:

Our mental health social work teams work within complicated legal frameworks to manage some of the more challenging and complex risks for people and their communities, balancing human rights and protection.

The teams provide a key counterbalance and social perspective to often very medical or clinical views about individuals. Working in partnership and collaboration with health colleagues in the community and mental health hospital settings ensures a holistic, rights-based response.

About The Job

Salary: £43,516 to £46,549 per annum
Working Pattern: Full-time (37 hours per week)
Contract Length: Permanent
Location: Based at either Bognor Regis and Horsham, some flexibility in base location and to be agreed with successful candidate.
