This unique role is based with the City & Hackney Rehabilitation and Recovery service, an integrated health and social care community mental health team. This friendly and dynamic team are based at Primrose Square, E9.
The team, who have been recognised at a national level for their innovative work, provide care to a caseload of service users with severe and enduring mental health needs living in Hackney. In addition, the team have a specialist role in supported accommodation for service users with mental health needs. This role of MH Supported Housing Panel and Placements coordinator is integral to this specialist work that the team undertakes.
The team provides:
oversight to whole MH accommodation pathway to ensure the best use of a limited resource and to ensure the needs of the service user align with the support on offer.
specialist reablement input to facilitate moves to more independent settings.
advice and support to colleagues across City & Hackney Services via in-patient in reach & weekly accommodation advice sessions.
regular support and advice through monthly sessions to all the commissioned providers in order to prevent placement breakdown and plan for move on.
The team also facilitates a fortnightly housing panel with colleagues from health and social care.
In this role you will be expected to be the first point of contact for MH panel, to process and allocate referrals in preparation for presentation to panel or pre-panel meetings, accurately record panel discussions and disseminate notes to relevant parties, adhering to data security protocols and agreed timescales.
You will work closely with LBH Finance and supplier management to ensure coordinated financial management of all placements and accurate recording / set up of payments. Using and developing existing management information systems you will contribute meaningfully to financial forecasting and performance reporting, and in relation to the panel’s activity and financial set up of placements / care packages
You will be expected to collect data in relation to, and administrative monitoring of, placements and review activity including tracking of actions and deadlines (working closely with Rehab senior practitioners and managers).
You will be required to take quality minutes of a number of high level, multi agency meetings and committees, and keep appropriate records.
You will also be required to provide a source of expertise on MOSAIC throughout the directorate, providing peer support to colleagues as appropriate.
Excellent organisational skills are required for this role, as well as the ability to design, implement and maintain office systems. An excellent knowledge of Microsoft office programs in particular Word, Excel and Access are required, combined with experience of working in a performance related environment.
