Liverpool City Council is looking to recruit Social Workers to a number of exciting opportunities within Adult Services and Health, assessment and care delivery; to be based within our community mental health (CMHT) and neighbourhood teams, as well as the newly-created, Care Home Team. The successful applicants will be required to contribute to the operational delivery of an effective Adult Social Care Service as part of an integrated team responsible for safeguarding, support planning, continuously improving outcomes and promoting the individual wellbeing of vulnerable adults /older people.
The successful Social Worker will conduct strengths based assessments, plan and co-design individual support plans and ensure that regular monitoring and reviews of care plans are carried out. They will be responsible for social work provision including managing own caseload whilst managing the reputation of the Department and Council and complying with national, and service, procedures and guidance
Social Workers hold, and effectively manage, a complex and varied caseload with appropriate supervision, guidance and support, reflecting the function of the team and Adult Social Care in accordance with policy, procedures, guidance and legislation.
For each case, the Social Worker will undertake work within legislative frameworks including the preparation of written reports, statements and representations to the judiciary at Court hearings and applications to the Court Of Protection.
You, as the Social Worker, will be accountable for complex decision making in relation to caseloads, ensuring you employ professional judgment, evidence based tools, critical reflection and analysis throughout.
Please note that as there are vacancies within both the CMHTs and the neighbourhood teams, your preference will be explored at interview and confirmed at the offer stage. All vacant posts are permanent and full time (35 hours per week).
