Islington is working hard to offer holistic and joined up care for people who with health and social care needs. As part of our work to make sure that the local health and social care system works for people we are looking for a senior practitioner who will work across the local authority and the local mental health trust to support colleagues to offer a holistic and personalised experience to people who need support.

Islington is part of NHS England’s Personalised Care programme. As part of this programme we are piloting a service called Choice and Control which offers people support from a peer coach to focus on their goals and what is important to them, and can give them access to a wider range of services to meet their goals.

The Choice and Control Lead will be working alongside teams within the local authority and mental health trust to identify people who would benefit from a more personalised and integrated approach and will support their colleagues to ensure that reviews and assessments take into account a person’s whole life. The lead will also work closely to co-ordinate and liaise with services from different organisations and act as a champion for personalisation across the local health and care system.

As well as being a qualified social worker, you will have significant relevant experience in the appropriate discipline allowing you to lead by example and set high standards for best practice.

The post holder will also be required work across multiple locations in the borough.
