The Transition Team, is based in Runcorn Town Hall , covering both Widnes and Runcorn. It is a small but dynamic and vibrant borough wide service for young people aged between 16 and 25. Although we sit in Adult Social Care, our journey with the people we support starts much earlier, working with our colleagues across Children’s services, health and Education. The Transition Team sits in a unique and favorable position of being able to develop positive relationships with young people and their families.

You will be passionate about working with young people with learning disabilities and physical and sensory difficulties, challenging discrimination whilst promoting independence.

We are looking for people who work with a strength-based approach, are motivated, flexible and who have high professional standards alongside sound knowledge and experience of current legislation. An applied knowledge of The Care Act and The Mental Capacity Act and experience of undertaking safeguarding enquiries is essential.. Our ASC workforce strategy supports our commitment to manage our talent and build succession plans for key areas, while investing in specialist training and development for our workforce.
An opportunity has arisen for a permanent 37 hour social work position.
You will have a recognised professional qualification Social Work Degree, MA, Dip SW or equivalent, good interpersonal and communication skills a flexible approach and a willingness to continually improve your practice; in addition you must hold a full driving licence.
You will receive a car user allowance and a flexible working week. You will receive up to 33 days annual leave per annum and a commitment to your on-going professional development.

If you are applying for this position as a Newly Qualified Social Worker, you will be expected to undertake the ASYE programme. Continued employment in the role is subject to successful completion of the programme.
