Are you an experienced Adult Social Care Worker? Are you looking for the next step in developing your skills, knowledge and experience within the Single Point of Access team (SPA)? We are looking for an Adult Social Care Worker to join an experienced team, comprising of Social Workers, Occupational Therapists and Adult Care Support Workers.
This is a challenging role where you will screen incoming referrals with the view to immediate resolution or triage on for further assessment. The role will provide you with the opportunity to work within an integrated health and social care team, whilst developing your skills and experience, you will be supported by Senior Practitioners and you will have access to appropriate training and development for the role.
The successful candidate will be joining the team at an exciting time in its development and it is hoped that you will make a positive contribution to the development of the Triage Service within the SPA. North Somerset established its Integrated SPA in 2009, providing timely advice, information and triage of referrals received for Adult Social Care and Health Services. Working within strengths-based, dynamic service delivery model, we are now ready to continue to develop the service, to ensure we are achieving excellent outcomes for residents of North Somerset. SPA is a diverse team comprising of Health and Social Care professionals and support colleagues.
We are now looking for a confident Adult Social Care Worker, with good time management skills, who can work in a quick moving environment to make sound evidenced based decisions to meet individual’s needs, efficiently and effectively.
