Portage is a home visiting service who work in partnership with parents/carers of pre-school children with a development delay or disability. Lewisham Portage is ever evolving, tailoring our service to best meet the needs of our children. We work to a key working model for all of our children with complex needs and their families as well as supporting our children on their transition into early year’s settings. Portage are often instrumental in requesting Education Health and Care plans where necessary and provide regular reports and evidence to support the application and assessment process.
Portage Home Visitors promote the welfare and safeguarding of children and young people.
Lewisham Portage is a well-established service in a vibrant, diverse community. Although we are home visitors we are based at Kaleidoscope Children & Young People Centre which is a multi-disciplinary base for community services for Children & Young People. We cover the whole of the Lewisham borough both in the home and supporting transitions into early year’s settings.
