We are looking for an experienced social worker to join the Mental Health Reablement Team. We provide short-term interventions to adults with mental health issues, and aim to prevent, reduce or delay long-term needs via the use of a targeted, goal-focussed period of support.
Working in a task orientated and strengths-based way, the successful candidate will work with service users to focus on increasing confidence and ability in areas of their lives that may have been impacted by mental illness. This will often involve hands-on work with service users within the community or their own homes, as well as with people leaving psychiatric hospital.
Following their period of Reablement support, service users will be assessed under the Care Act 2014 and an eligibility decision will be reached regarding ongoing social care support. The successful candidate will also complete safeguarding enquiries and mental capacity assessments as appropriate, as well as participating in duty work and referral triaging.
