Rate of Pay: £10.55 - £11.53 per hour

We are seeking an individual who is looking for a new challenge to lead on the delivery of the new Home to Hospital Support Service in Elmbridge. This role involves supporting patients with non-medical support who have been discharged to return safely and live independently in their own home for as long as possible. This will be through various means of support including a home check, providing an emergency pack of food, advice on hydration, sign posting to Community Support Services and the voluntary sector support in the borough. Initial contact is made with discharged patients and can continue for up to 6 weeks. You may be required, occasionally, to pick up discharge patients from hospital and transport them home.

 

To be successful in this role you will need to have a back ground in the community and or care settings and understanding of the needs of older, frail, disabled and vulnerable adults. Knowledge of a range of services available within the Borough to support the discharged patients would be advantageous but not essential. Communication is key in this role and you must be a clear and confident communicator on a one to one basis to a variety of clients.

 

To be successful you will need to be able to manage your own work load and time well ensuring each referral is supported from initial referral to the service until the support is closed down after 6 weeks (or earlier if appropriate). You need to be confident using computers and completing notes and spread sheets to maintain records accurately. You must be able to work independently.

 

The Council has received funding for a 12 month period with a concluding review at the end to determine any future arrangements. The area covered by this post is East Elmbridge and will involve travelling to the homes of discharged patients to complete home assessments however telephone support, in some cases, may be sufficient. You will need a full driving license with access to a vehicle on a daily basis. You will also be required to undertake an Enhanced DBS disclosure.
