The Adult Disability Service provide support to adults with a learning or physical disability, autism and long-term conditions. An exciting opportunity has arisen for 2 Advanced Practitioners to join the service to work alongside our Continuing Healthcare (CHC) colleagues across Hertfordshire. You will need to be a registered Occupational Therapist (HCPC) or Social Worker (SWE) and have knowledge and experience of applying relevant legislation. We are looking for dynamic workers who have a commitment to supporting our wide range of service users and local communities and can proactively support other workers with learning and practice.
The post holders will focus on working with CHC colleagues to complete assessments and Decision Support Tools for people within the community. You must have an excellent understanding of the CHC Framework and how to apply this in practice. It is important that you have the skills to develop positive working relationships with both CHC and community health teams to ensure good integrated working.
