We are looking for an Independence Support Assistant to join our Portsmouth Adult Mental Health Recovery Team. It is an integrated health and social care team, providing support for people with severe and enduring mental illness. The team consists of Occupational Therapists, Mental Health Nurses, Social Workers Support Workers Psychologists and Psychiatrists. The post holder will provide skilled support to maximise independence and recovery in the community.
You will ensure people eligible for social care assistance have the maximum choice, control and power over the support services they receive to achieve the best possible quality of life and level of independence.
You will work within Adult Social Care eligibility criteria to provide assessment and advice to clients and carers, to manage practical tasks through a problem solving approach and to provide creative solutions which meet peoples’ needs and reduce risk.
You are to assist clients in the process of self directed support & to ensure that all Portsmouth City Adult Protection Policies are adhered to and concerns are raised in accordance with these policies
You will be expected to work 9:00am till 5:00pm Monday to Friday. A degree of flexibility is required to ensure safety of clients and therefor some evening work may be necessary and time will be taken back in lieu.
From time to time you may be asked to work at a different base to cover operational needs.
