Halton Borough Council is offering a unique opportunity for an experienced Social Worker to become part of the Hospital Integrated Discharge Team (based in Warrington Hospital). You will have achieved ASYE and submission of a successful ‘Development Portfolio’ or equivalent learning and practice experience demonstrating capabilities commensurate with the relevant level of the Professional Capabilities Framework.

Halton Borough Council’s expectations are high and you will need a strong commitment to providing first class person centred care to Halton residents over 18 and their carers.

The Social Worker role involves discharge planning and you will have experience in completing assessments and care planning to facilitate safe and timely discharges. You will be passionate about working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties), whilst challenging discrimination and promoting independence. You will ensure that your work and interventions are person centred and that you support people with lived experienced to make their own choices and decisions and have choice and control in their day-to-day life. This also involves supporting people to be involved in their local community and developing and maintain relationships, including liaising with the voluntary sector to provide relevant support.

The Hospital Discharge team works closely with the Halton Intermediate Care and Frailty Service (HICAFs) to expedite discharges into a Reablement Service and Intermediate Care Bed base to support recovery following a Hospital admission, prevent and/ or reduce need for long term services and allow time for further assessments to take place in the community. An understanding of Intermediate Care would be an advantage for this role.  

You will have knowledge of relevant social work legislation such as the Care Act 2014 and Mental Capacity Act 2005; having knowledge and some experience in risk assessments and risk management.

You will be a team player and be dedicated to Multi agency team working within the hospital setting. You will be able to manage and organise your own case load to ensure that people are discharged in a timely manner when medically fit to leave Hospital.

You will have a strong personal commitment to anti-oppressive practice and be able to demonstrate your commitment to challenge discrimination through your daily practice.

Applicants must have good interpersonal and communication skills, a flexible approach and a willingness to learn new skills and continually develop. Ideally you will have good IT skills.

In addition you must hold a full driving licence and complete a DBS application.
