The Integrated Community Support Commissioning Team is responsible for commissioning a range of social care services for vulnerable adults in Hertfordshire to support them to remain living independently at home. This primarily relates to Older People but also includes adults with a Learning Disability, Physical Disability or Mental Health condition.
We are recruiting to 2 full-time Monitoring Officer positions on a fixed term/secondment basis, due to team members taking maternity leave from September 2022.
Monitoring Officers are key relationship holders with external homecare provider partners and are responsible for monitoring the quality and effectiveness of commissioned services, ensuring contractual compliance and that services are being delivered at the required standard. This involves carrying out monitoring visits to provider premises, along with analysis of data and intelligence received. You will build strong relationships with operational colleagues and stakeholders to share intelligence regarding provider performance and ensure monitoring processes are followed in a timely and effective manner. You will work with providers to support the wider health & social care system to ensure quality homecare services are available when and where needed, preventing avoidable admissions to hospital and facilitating timely discharges.
With an ability to work under pressure and juggle priorities, you will continuously seek improvements in the quality of commissioned services and work with providers and stakeholders where concerns are identified.
The ability to work collaboratively across commissioning and operational teams, and to develop and maintain strong links to the provider market and partner organisations are vital to this role, along with passion to deliver quality homecare services to vulnerable adults in Hertfordshire.

Applicants should note, that the Council is developing a new Quality and Performance team, who will be responsible for undertaking quality monitoring of provider services. It is not yet known whether these 2 posts will transfer to the new team. Should this be the case, however the job outline and pay structure would remain the same.
