The social work position is in the Discharge to Assess team based at Ely house in basildon. The Discharge to Access Team works closely with our hospital teams and health colleagues. The role of the team is to support the health discharge to assess pathway from hospital. We cover the whole of south east which includes two hospitals. All adults are assessed as to whether they can return home on their existing care plan or require a discharge to assess pathway. The team pick up any case on the discharge to assess pathway ensuring that the assessment takes place away from the inpatient setting.
The team works with these individuals for a period of up to 6 weeks completing assessments to try an maximise their independence to ensure the adults outcomes are met in whatever environment that is .
The team works in a multidisciplinary approach to the care planning.

The role requires you to be mobile throughout a wider operational area including travelling around the county of Essex and beyond, transporting service users and their belongings and to be able to do so in an emergency. Driving licence and car or the ability to meet the mobility requirements of the role through other means is essential.
As part of employment with Essex County Council (ECC) should your role require a DBS check you will be required to register with the DBS update service. The DBS update service ensures all registered DBS certificates are kept up to date and allows authorised parties to view the status of your certificate.
