We are looking for proactive and dynamic individuals to join our Social Work teams in an Integrated Community Health and Social Care service, working across the Wirral Peninsula.

 

Main duties of the job

 

Full and part time vacancies are available in:

  • Community (Integrated Community Care Teams)
  • Integrated Discharge Team

You will work with people aged 18+ who may have multiple conditions, learning disabilities and complex care needs as part of a multi disciplinary team in a community setting.

To join our Social Work teams you will:

  • Be a registered Social Worker with Social Work England
  • Have BIA qualification or  Practice Educator award
  • Have proven experience of working with vulnerable adults with complex needs, families and carers
  • Have applied knowledge of The Care Act 2014/ Mental Health Act 1983, Mental Capacity Act and other appropriate regulatory frameworks and statutory guidance relating to adults
  • Apply social work ethical principles and values to guide professional practice, model and support others to reflect on and manage the influence and impact of own values on professional practice
  • Be committed to delivering a high standard of care and promoting professional standards
  • Be an excellent communicator who will inspire, support and lead others
  • Advocate for members of our community, identifying changes in need and planning for their current and future care

Qualifications & Training

Essential criteria
  • Social Work qualification e.g. Degree in Social Work; DipSW or equivalent SW registered qualification
  • Be registered with Social Work England and able to evidence this
  • Evidence of continuing professional development and able to evidence this
  • Relevant post-qualifying/vocational training courses. (Best Interest Assessor or Practice educator)

Experience & Knowledge

Essential criteria
  • Experience of completing assessments, support plans, risk assessments, reviews, and court reports and generating these through information, technology.
  • Applied knowledge of The Care Act 2014/ Mental Health Act 1983, Mental Capacity Act and other appropriate regulatory frameworks and statutory guidance relating to adults (PCF1)
  • Proven experience of working with vulnerable adults with complex needs families and carers
  • Ability to fully consider the dimension to own activities, monitoring expenditure through reviewing individual needs ensuring true value formoney is delivered (PCF8)
  • Experience in applying the Assessment Framework and other relevant assessment tools (PCF5)
  • Experience and ability to demonstrate skill in undertaking asset/strengths based approach to assessment and an outcome focused approach to support planning. (PCF7)
  • Ability to support and lead others, demonstrating excellent organisational skills and ability to prioritise and manage fluctuating caseloads/servicedemands producing work to a high standard within time scales.
  • Ability to effectively undertake partnership working with adults with care and support needs and their families/carers/advocates and other agenciesinvolved in their care.
  • Ability to recognise and promote safety, health, wellbeing and emotional resilience for both self and colleagues.
  • Able to demonstrate experience across a range of statutory work including assessment, planning, safeguarding and court work
  • Understanding of confidentiality and data protection requirements
