The all age Transforming Care Team has an exciting opportunity for a Complex Care CMO looking to further their career within the 0-25 Transforming Care service.

You’ll be working at the forefront of best practice to deliver personalised support to vulnerable adults with complex care needs such as autistic spectrum disorders and learning disabilities.  This will include complex inpatient discharge planning and supporting those at risk of inappropriate mental health hospital admission.

It is essential you understand Transforming Care national programme and able to deliver on NHSE requirements locally. You will have experience working with inpatient clinical teams, community health specialists and working with NHSE and bed commissioners.

The role includes submitting data and bidding for funding/property, often under great pressure. You will present data to senior managers to enable and effect change and inform evidence-based service development.

You’ll be a key team member responsible for complex and personalised service finding, creative and bespoke care panning and its essential to have experience in this area including commissioning on systems.

You will work in partnership with the children’s Transforming care team, the Integrated health care commissioning team and others including social care, inpatients units and community health teams to share and develop knowledge to enable smooth transition pathways.

With experience in commissioning/ delivering complex partnership projects, you will work collaboratively to develop an improved offer of support; improving outcomes for young adults, adults, their families and the professional systems that support them. Your work will involve collaboration with a range of stakeholders through co-production, engagement and participation, using your initiative and leadership skills to find solutions to problems in partnership, and be accountable for driving forward service improvements.

You will have line management responsibility of a TC senior service finder, fulfilling professional development, supervision and workflow of others.

You will demonstrate your excellent communication skills and work in a personalised way, co-produce with stakeholders including clinical teams, commissioners and the voluntary sector, parent carers and of course adults themselves.  You will be expected to make autonomous decisions, be able to Smart Work and visit out of county placements/hospitals.
