Permanent Full Time Post – 37 hours (part time may be considered)

An exciting opportunity has arisen for an Assessor to join our WISH (Wokingham Integrated Social Care and Health) service. The post holder will work within an established multi professional statutory assessment team and will undertake assessments for people in the community in the early stages of the Adult Social Care Pathway using a Strengths Based Approach to establish eligibility for Adult Social Care Services in line with the Care Act 2014 Framework.  You will also identify people at risk and take initial Safeguarding information as part of the role.  The role involves working with adults with a broad range of needs including people with learning disabilities, physical disabilities, older people, and people with dementia. It also involves completing Carers Assessments and providing information, advice and support to Carers. You will also take part in the duty rota.

You will need to be flexible and be able to prioritise and manage your time effectively.  A car driver
is essential as we cover a rural as well as urban area and there is very limited public transport.

We are a very supportive team and offer a structured induction programme, regular supervision, support and opportunities for training and development including access to the social work apprenticeship programme. We would be keen to support the successful applicant to undertake the Social Work apprenticeship but this would be subject to further interviews and the University entry requirements
