We have an exciting opportunity for a Senior Social Worker to join our highly successful Community Discharge Hub. The post holder will work within an established and committed multi professional statutory team and work closely with the linked Pathway 3 (Integrated Discharge Team). The post holder will have the experience and knowledge of completing person-centred and strength-based assessments within legislated timeframes. You will be working alongside multi-disciplinary internal and external colleagues and teams including health professionals, service providers Continuing Health colleagues, advocacy services, voluntary services and so forth, so must demonstrate the ability to undertake seamless and effective multi-disciplinary practice.
The post-holder will demonstrate a ‘home first’, empowering and least restrictive approach to practice, and recognise the need to consider returning to community-based living, irrespective of individuals being assessed whilst cared for in Pathway 3 care setting. The post holder will provide individuals with support and advocacy which utilises the post-holder’s expertise, in order to empower people to achieve their personal outcomes and choice and control over day-to-day life.
The team is the recipients of Pathway 3 discharges and assesses service users discharged from acute and community hospitals, and intermediate care units under Pathway 3 Discharge to Assess. The team routinely undertake assessments in line with the Care Act 2014, within the timeframes dictated by Hospital Discharge and Community Support: Policy and Operating Model. The post-holder will demonstrate an ability to assess risk, including positive risks, and facilitate long-term, person-centred and least restrictive care planning. The post-holder with demonstrate an understanding of fiscal responsibility and recognise the need to maintain flow in the community care settings, demonstrate fiscal responsibility and manage cases within timeframe. The team provide a service for all adults over the age of 18 years, with broad and diverse needs, including dual diagnosis. The post-holder must be confident in safeguarding related practice, identifying, and working with informal carers, undertaking mental capacity assessments and convening multi-disciplinary meetings/ best interests’ meetings.
The team is a fast paced, front-line team which although challenging, provides a diverse practice experience and with high levels of support. You will hybrid working and work both remotely as well as being based at Medway Council’s Gun Wharf offices. There may be occasions depending on service demand you may be required to attend Medway Maritime Hospital, Gillingham.
