Our Adult Social Care Central Locality is seeking a new Locality Manager to lead the three Teams within the Locality and includes the line management of two Locality Team Leaders, the Transitions Team Leader and the Administration Officer. The Locality is based on the site of the West Berkshire Community Hospital, close to the community it serves in Thatcham and the surrounding villages.
The successful candidate will be promoting close working relationships with colleagues in Housing, with local GP surgeries and with voluntary sector agencies in the area. You will be responsible for ensuring that the council resources are used effectively to support vulnerable residents and you will contribute to the development, implementation and monitoring of the service plan and continuous improvement of the service. You will also be leading on the development of the Transitions team for Adult Social Care, working closely with colleagues in Children’s Services, Education, Health and other external agencies to ensure a coordinated approach to the transition of vulnerable 14 – 25 year olds into adulthood.
The post holder must hold a relevant professional qualification - either Social Work or Occupational Therapy - and will need to have relevant experience as a manager.
