Senior Social Worker, Integrated Care
Grade 9, £38,890 - £43,857 per annum
An exciting opportunity has arisen for two Senior Social Workers to join our services in Wokingham’s Integrated Care and Support (WISH) team. As part of this role you will be at the heart of our integration with Health, in developing a strong relationship with our health colleagues. We are seeking motivated, autonomous and creative applicants to drive relationships, build networks and support joint working between our services.
With Health, Adult Social Care are running a number of innovative programmes to promote integration and develop proactive and preventative work. We are therefore looking for applicants to progress and maintain excellent relationships and links with Primary Care Networks, GP Practices, Community Health Services, hospitals and Voluntary Care Services (VCS).
The post will provide opportunities for joint working, training and problem resolution and promote a shared knowledge of working practices and access to information and support. Wokingham Borough has established Multi-Disciplinary Team meetings involving Health, Adult Social Care and the VCS. Part of the role will be to chair the MDT meetings and to actively contribute to their ongoing development and success.
The posts will be based within WBC Adult Social Care, one post within the long term services and the second post within hospital discharge, in order to embed this way of working more broadly. The post holder will also be required to manage a small caseload, generated from MDT referrals and to provide supervision to a maximum of two social workers.
The post holder will be a Qualified Social Worker and registered with Social Work England. The applicant must have experience of working in Adult Social Care. with good knowledge and experience of working within all relevant legal frameworks. The ability to work in a busy, fast-paced environment and to be able to plan and adapt to change is essential.
