In summer 2020 a new vision for the Day Opportunities Service was developed. The new vision and approach have huge potential to improve the lives of those living with learning disabilities and autism supported by the service, this means the service now focuses on what is possible, rather than what isn’t. One that provides independence, prioritises choice and dignity, supports health and wellbeing, preventing social isolation and loneliness. One that builds resilience, is person-centred, evolves with each individual, and offers fulfilled lives and a brighter future.  If you can lead and motivate staff who want to make a difference this is the job for you…

 

About you…

  • Qualified to NVQ Level 3
  • Experienced in leading and supporting a team operationally
  • Have a positive attitude to disability and an understanding of the purpose of advocacy
  • Experience of Person Centred Planning and Health Action Plans
  • Good verbal and written communication skills
  • Able to prioritise own and others work, and motivate staff members to be the best they can be
  • Able to work flexibly as part of a rota to include weekends, bank holidays and evenings


About the role…

  • To Coordinate and facilitate service provision supporting people who have been assessed of having a need for support, promoting their independence and enabling choice and control
  • To promote social inclusion, reducing isolation and contribute to service development to meet both individuals and service outcomes
  • To maintain the staff rota to ensure adequate support is provided, coordinating annual leave, training and sickness cover amongst the team
  • Arrange, coordinate and contribute to multi-disciplinary planning meetings for individuals to ensure an integrated coordinated health, social care and community approach to service delivery


The vision for our services is for a more personalized service with more choice for all providing a 24/7 service with health and wellbeing at the heart. Most importantly, it is now a service that builds independence and evolves with each individual. It’s all about putting life into years!

 

Mandatory Vaccinations…

The Community Support Services Coordinator position has been identified as needing to comply with the mandatory vaccinations legislation. If you are the successful candidate, you will need to provide evidence that you have received both first and second dose vaccinations or evidence of medical exemption.

 

The successful Community Support Services Coordinator will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.

 

The Community Support Services Coordinator interviews will be held in November 2021,
