Senior Social Worker/ Senior Occupational Therapist- Duty

An exciting opportunity has arisen for a qualified Senior Social Worker or Occupational Therapist with full Social Work England/ Health & Care Professions Council registration to join our Duty Assessment Team. The post holder will work within an established multi professional statutory assessment team, be responsible for the supervision of team members and will take a lead role in managing the responsiveness of the Adult Social Care Duty Team. This will involve providing a range of appropriate, professional support giving information, advice and signposting to adults with care and support needs and their carers and maximizing use of social capital and community services. The role will require you to assess the needs of individuals and their carers using a Strengths Based Approach and decide, with regard to the level of risk, how to meet the identified needs on a short-term basis with focus on prevention and reablement. The role provides the opportunity to work with adults with a broad range of needs including people with learning disabilities, physical disabilities, older people and people with dementia. Working closely with our voluntary care sector to support individuals to continue living as independently as possible within their communities will be an integral part of this role.
We are a very supportive team, and offer a structured induction programme, regular supervision, ASYE, and good opportunities for CPD.
You will need to be flexible and be able to prioritise and manage your time effectively. A car driver
is essential as we cover a rural as well as urban area and there is very limited public transport.
