Job Duties:

WE ARE LOOKING FOR COMMUNITY CARE OFFICERS TO JOIN OUR HERTS VALLEYS INTEGRATED HOSPITAL DISCHARGE SERVICE, POST HOSPITAL REVIEW TEAM. 

What does the role involve? 

The service supports an integrated approach to managing the care of people who require support after a hospital admission.  The role will involve working alongside Occupational Therapists, Social workers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.  

We promote personalisation focusing on the wellbeing of the individual.  Through care planning we strive for increased independence, choice and control by enabling people to direct their own support.  You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.

What is required? 

You will need to have knowledge and experience of relevant legislation.  You should be able to demonstrate a commitment to supporting people with the right care and information at the right time to help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to within our Connected Lives Framework (Your connected life | Hertfordshire County Council).

To improve outcomes for service users, the Integrated Discharge Service supports a 7-day service.  You will be asked to be part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after. 

The nature of this role will be instrumental within the Discharge to Assess model completing assessments of need, planning and delivering services and reviewing outcomes with the individual, their personal networks and support providers within a focus of ‘Home First’.

English Fluency: The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post.
