There is an exciting opportunity to join the Transitions and Review Team in Adult Social Care. We are small team within Adult Social Care, who are looking to expand and develop the work we do, in line with the Adult Social Care Improvement Programme. We currently have vacancies for qualified Social Workers, an Occupational Therapist and Social care officers.

The team works in 3 key areas of adult social care:

Care Act Reviews for existing social care clients. You will be working closely with our commissioning, brokerage and finance colleagues, working in partnership with our providers to ensure the best outcomes are achieved for the people we support.

Transitions into adulthood for those young people known to Children’s Services. We are working closely with Children’s Social Care to support those Young People who will be eligible for support from Adult Social Care, ensuring that they are supported appropriately to allow them to reach their full potential.

Supporting the discharge of those people under the Transforming Care agenda. We work closely with our Health Colleagues to ensure the appropriate services are identified to enable safe discharge back into the community for this cohort of people.
