Job Title: Community Care Officer - Herts Valleys
Location: Apsley, Hemel Hempstead
Salary Range: £23,080 - £30,451pa
Hours: 37 hours per week

Adult Care Services are looking to recruit a highly motivated Community Care Officer to join the Herts Valleys Older People’s Team. You will need to demonstrate an interest in supporting older people with age related frailties or dementia and their carers. You will be joining a busy and dynamic team providing social care support to residents of West Hertfordshire.
You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being. You will keep the service user at the centre of all activity and address assessed needs within the Care Act and other legislative framework. You will work autonomously within agreed timeframes and manage frequently changing circumstances with the support of your line manager.
It is vital that you share in our dedication for equality of opportunity. You will need to demonstrate excellent communication skills and be able to work closely with partner agencies such as community navigators, the community nursing team, St Albans District Council, Age UK and other local voluntary organisations
You will be competent at keeping accurate manual and computer records and will be able to interpret and prioritize these as appropriate.
