We are looking for an experienced qualified social worker or occupational therapist, ideally with experience of working in an integrated setting.  This is an exciting role working closely with health colleagues across 2 NHS care networks to support prevention of admissions to hospital and enable people to remain in their own homes where possible.

We are looking for a dynamic worker who has a commitment to supporting the local community and building links and networks with our partners in health.  You will need to be a registered professional and have knowledge and experience of relevant legislation.
