An opportunity has arisen for the role of Advanced Practitioner in the St Albans Adult Disability Team.

You will be a SWE registered Social Worker or Occupational Therapist and preferably either an AMHP, BIA or Practice Educator.

The role will require you to lead on peer development and practice, support and supervise staff to develop their skills and knowledge in line with related legislation including The Care Act and Hertfordshire’s Connected Lives principles to contribute to the ongoing development of the service.

St Albans Adult Disability Team, work with adults aged 25+ who have varied complex needs and are able to manage with the right kind of support. We have social workers, occupational therapists, and learning disability nurses within the team to ensure a multi-disciplinary approach to the care of the people we serve.

You will be responsible for delivering high quality statutory social care including safeguarding, assessment, supervision and care management functions as well as developing innovative, creative community solutions. You will be pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families and carers.

Our service is continually developing to ensure a true delivery of personalisation and citizenship through our Connected Lives principles. We are looking for someone with a passion for empowering and enabling citizens, who can lead by example and support staff to promote community inclusion and develop independence.

The role is part of an innovative and supportive service that strives to ensure that Hertfordshire’s people don’t just have their needs met but are listened to, supported to be respected and included as citizens within our local communities.
