About the Post:
Barnsley Adult Social Care have an exciting opportunity for an Assistant social care practitioner, working with the Hospital social work team.
The roles involve working with and supporting adults over the age of 18 years, who are in hospital, Intermediate care, or have recently been discharged home.
You will need knowledge of the legal frameworks that guide and influence the work of Adult Social Care. The role will include carrying out assessments and reviews under the Discharge to Assess procedures, Care Act 2014, and working with the Making Safeguarding Personal agenda.
A multi-disciplinary approach is a key part of the role, including being part of assessments where continuing health care is of consideration, working closely with health colleagues to promote safe and timely discharges, assessing service users in hospital and at home, whilst working closely with Reablement services to ensure strengths-based outcomes are achieved.
