We have an exciting opportunity for a qualified Social Worker to join our WISH (Wokingham Integrated Social Care and Health) service in the recently expanded Health Liaison Team. The post holders will work within an established and committed multi professional statutory team and work in the field of hospital discharge in the Wokingham area, undertaking strength based assessments for people in acute or community hospitals and requiring discharge on a “home first” basis, working alongside our colleagues in health.
The team establishes eligibility for Social Care services in line with the Care Act (2014) framework and will involve meeting immediate need to manage risk and to ensure a timely discharge from hospital to the community, currently under the rapid discharge guidance of Discharge to Assess model. The Team will work with adults with a broad range of needs including people with learning disabilities, physical disabilities, older people and people with dementia. It also involves completing Carers Assessments and providing information, advice and support to Carers. We are based at Shute End in Wokingham, but during this time of home working and use of IT systems such as Teams and Outlook are part of our daily working practices.

You will need to be flexible and be able to prioritise and manage your time effectively. A car driver
is essential as we cover a rural as well as urban area and there is very limited public transport.
