About the role…

The Senior Practitioner, Long Term & Review is responsible for the following:

  • Supporting the development of the team, with responsibilities for dealing with more complex cases, including assessment and appropriate management of risks
  • Working with individuals, families, carers and communities to help them make informed decisions
  • Supervising/mentoring team members ensuring policies/practices are implemented and all planning and review activity is completed effectively

 About you…

  • Qualified Social Worker and registered with Social Work England (SWE)
  • Experienced in managing a complex case load with supervisory experience
  • Able to support a team to be the best it can be, promoting integration with health and managing change
  • Experienced in partnership working

 The role of the Senior Practitioner, Long Term and Review Team is to supervise Social Workers and Care Managers holding complex cases of adults with care and support needs, including people with learning disabilities.  You will support the Team Manager and be part of a progressive integrated team, who are ensuring personalisation is central to their work.

The market supplement is a discretionary payment paid to qualified Social Workers.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us plus we pay for Social Work England registration.

The Senior Practitioner, Long Term & Review interviews will be held on 23/24 August 2021.
